Community Health Workers act as advocates on behalf of their participants. 

Services are ongoing for individuals or families who are needing one on one assistance with applying for benefits, finding medical insurance, needing food, clothing, transportation or housing, support during pregnancy, finding a job, childcare, or a specialist, etc.
